The university provides reasonable accommodations to qualified job applicants and employees (including teaching and research faculty, classified and exempt professional staff, and student employees) with a disability in order to enable the employee to perform the essential functions of her or her position.
The university provides reasonable accommodations to employees experiencing conditions related to pregnancy and childbirth.
An employee who would like to request a reasonable accommodation due to disability or pregnancy, in order to perform the essential functions of their position, may make a request to the employee’s supervisor, or directly to ADA Compliance. If an employee makes a request to the employee’s supervisor, the supervisor should refer the employee to ADA Compliance, while also contacting the ADA Compliance unit directly to communicate the request.
When possible, employees should submit accommodation requests in writing, either via email or by using the forms below. This shall initiate the interactive process.
The ADA requires that ADA Compliance keep medical information confidential. However, the law allows ADA Compliance to share information regarding an accommodation request with individuals who are considered to have a legitimate need to know. These persons can include supervisor(s), human resources personnel, first aid, and safety personnel, personnel investigating compliance with the ADA, and other persons considered to have a legitimate need to know. The law does not prohibit the person requesting the accommodation from voluntarily discussing their condition or medical information with others.
Employee Accommodation Forms
Request an employment accommodation through the ADA Compliance unit. An individual's healthcare practitioner will need to complete the medical information request form in order to provide supporting documentation.
